FIREWOOD SPA TERMS & CONDITIONS
Disclaimer: A credit card is required to book with Firewood Spa. Credit card confirmation is required to complete the booking, including but not limited to all 3rd party payments, prepaid services, gift certificates, gift cards and gift vouchers. A consent form is required to be completed by each guest prior to the scheduled appointment, for each service booked. The consent form is sent electronically, via text or email, we do not have paper forms at the spa. There are booking restrictions for 3rd party payments. Taxes and gratuity are not included in 3rd party payments and a credit card is required to book an appointment. 3rd party payments are for your first visit only and valid for new customers only, additional fees will apply if existing customer.
Weekend Charge/Ohio Sales Tax: A weekend charge of $25.00 will be added per service on Spa Treatments, Friday, Saturday and Sundays at Firewood Spa. This charge excludes Waxing Services. A state sales tax of 8% will be added to each service.
Expecting Mothers: Pregnancy must be disclosed at the time of booking, online, on the phone, in person or via third party, in order to discuss service options. Your appointment could be canceled if this information is not provided at the time of booking and a cancelation fee will apply, being 30% of the service price if we are unable to accommodate the service at the time of the scheduled appointment.
Disclaimer: All service times include 10 minutes for dressing and consultation. Any enhancements or add-ons are included in the duration of the service. If late for an appointment, the remainder of time will be given.

Eternal Flame Program: The Eternal Flame Program provides discounted spa treatments and wellness packages. NO Membership Fee Required, just purchase your next service or package at checkout and keep this price for all your future visits at Firewood Spa. Each service purchased will be valid for 30 days from the date of purchase. There is a $10 charge to utilize an expired service, as well as a $10 charge to gift a prepaid service. If you decline to pre-purchase at checkout standard pricing will apply for any future services.
Cancellation Policy: Cancel for free up to 24 hours before your scheduled appointment, otherwise you will be charged 30% of the service price for late cancellation and 50% of the service price for canceling the same day or not showing up for the scheduled appointment.
Refund Policy: Services are non-refundable, non-transferable and non-exchangeable. Under no circumstances will refunds be issued for services rendered. Gift Certificates and/or Voucher are non-refundable and not a valid form of payment for a no-show or cancellation fee.
